Self-Enrollment Directions

Thank you for your interest in My Trinity Health.

In order to self-enroll, you will need your Medical Record Number and have a valid email address on file with the health system.

How Can I signup?

If you know your Medical Record Number and you have provided your email address when you last registered, you can go to Self-Enrollment to complete the enrollment form.

If you do not know your Medical Record Number or you do not have a valid email address on file with the health system, you must visit the Health Information Management department to verify your identity. Once your identity has been verified, the Health Information Management department can enroll you in the portal.
