Skilled Care Center
The
goal of Trinity Skilled Care Center is to provide services which
in turn allow the patients to regain independence in daily living
and return them to their highest level of well being.
The Trinity Skilled Care Center, a hospital-based skilled and long
term care nursing facility is located in Steubenville, Ohio, overlooking
the Ohio River. Trinity Skilled is fully-certified for 50 beds and
was developed to meet the needs of patients who require additional
skilled care beyond the acute phase of illness or injury.
If you or a loved one require Skilled Care services after an acute
hospital stay, allow our professionally trained staff to care for
you in your home away from home.
Our highly trained health care professionals offer the following
services when your physician orders them:
• 24 hour professional nursing care
• Social services
• Physical therapy
• Occupational therapy
• Speech therapy
• Pulmonary rehabilitation
• Nutritional assessment and management
• Diabetes consultation
Admissions to the Trinity Skilled Care Center are by a direct physician
referral after a three day stay in an acute care facility. Patients
can be admitted immediately or within 30 days after discharge. Patients
must meet the Medicare criteria for admission to the Skilled Care
Unit. Skilled services include teaching and training activities
such as:
• Gait training
• Prosthesis care for a recent amputee
• New diabetic teaching
• Colostomy or ileostomy care
• Self administered gastrotomy feedings
• Use and care of braces or splints
• Any specialized dressings
• Pain management
• A program to improve activities of daily living, such as bathing,
dressing or eating
Additional services include:
• IV medication
• Treatment of grade 3 or worse, decubitus ulcer
• Nasogastric, gastrotomy and jejunostomy feedings
• Physical, occupational and/or speech therapies
Trinity Skilled Care Center has an ongoing activity program. Patients
are encouraged to participate in these enjoyable activities as a
group or individually. The atmosphere is informal and relaxing to
aid in the emotional well being of each patient.
Discharge planning begins on admission to Trinity Skilled Care
Center. Individualized goals are established with you, your family
and our highly qualified team of professionals. This team includes:
the attending physician, nurse, social services, dietician, physical
therapist, occupational therapist and speech therapist. This process
assures the continuity of care upon discharge from Trinity Skilled
Care Center.
