In Patient Physical Rehabilitation

Trinity’s Rehabilitation Center provides an integrated and interdisciplinary team approach to help patients who have physical impairments. Programs are available for patients with:

  • Strokes and other neurological disorders
  • Arthritis
  • Amputations, joint replacements and other orthopedic problems
  • Brain injury
  • Spinal cord injury

The newly renovated Center has 20 private rooms, each specially designed for rehabilitation.  Patients also utilize a home-like apartment to regain skills such as cooking, bathing and dressing. Physical Therapy, Occupational Therapy and Speech-Language Pathology areas have state-of-the-art equipment, and the therapists are highly trained rehabilitation specialists.

Meals are served in the newly remodeled dining area.  This area is also utilized for various social and recreational activities.  After discharge, patients may continue their rehabilitation programs in our outpatient rehabilitation center.

A van is available for community outings and home visits.  These trips provide opportunities for patients to practice their skills in their community, home and work place when appropriate.

Team Approach

With a team approach to treatment, a personalized rehabilitation program is designed to meet the unique needs and goals of each patient.  The team consists of the patient, his/her family, physicians, rehabilitation nurses, physical therapists, occupational therapists, speech language pathologists, psychologists, social workers, therapeutic recreational specialists, dieticians, respiratory therapists, case coordinators and vocational counselors.

Through a positive, personalized and interdisciplinary approach, the team helps patients regain physical, emotional, social and verbal skills needed to function at home, work and in their own community.

Patient and Family Involvement

Patients and their families actively participate in the therapy program which is designed to meet each patient’s unique goals and needs.  Each patient and family is encouraged to participate in the development of personal rehabilitation goals.  Family members play a crucial role in the success of the patient’s rehabilitation.  Family members will develop a sense of understanding and confidence in helping the patient meet his own needs through participation in family education programs offered by the rehabilitation staff.

At some point, family members may be encouraged to take patients out of the hospital in preparation for discharge.  The therapy staff will provide the necessary training to make this experience a productive part of the rehabilitation process.

What to Bring

A family member or significant other should be present during the admission process. The family member is asked to bring the following:

  • Loose fitting clothing such as jogging outfits and sweat suits
  • Non-skid shoes such as tennis shoes
  • Personal toiletries including shampoo, razors and make up
  • Medical insurance information and/or Medicare card
  • List of current medications

Visiting Hours

To reach the highest level of function and independence, patients must participate in all therapy sessions.  Visitors are asked to please observe visiting hours.

Monday – Friday 4:00pm-8:00pm
Weekends/Holidays 1:00pm-8:00pm

By providing an array of intensive rehabilitative services, the Center continues the mission of Trinity Health System, to make the highest level of quality care available to our community.

Inpatient Admission Criteria

In adherence to Medicare and accreditation standards, the following are basic criteria for admission to the Trinity Rehabilitation Center.

The patient must have one of the following medical conditions: Stroke, Amputation, Major Multiple Trauma, brain injury, congenital deformities, polyarthritis including knee replacements, spinal cord injury, orthopedic dysfunction or degenerative neurological disorders including multiple sclerosis, polyneuropathy, muscular dystrophy, Parkinson’s disease and Guillain Barre.

The patient must require an intensive rehabilitation program which offers a multidisciplinary team approach.  The patient must need close medical supervision, 24-hour rehabilitation nursing and participate in three hours of physical, occupation and/or speech therapy daily.

The patient must have the potential for achieving improvements in activities of daily living.
