Diabetes Education
The Diabetes Outpatient Education Program at Trinity Health System
is specifically designed to aide the patient with diabetes to make
responsible decisions in the care of their disease.
The program is offered in seven sessions.
Content includes:
- Disease Process
- Two nutritional sessions, by a Certified Dietician
- Medications
- Acute Complications/sick days, travelling
- Exercise, monitoring
- General health care, chronic complications
Prior to entering the teaching sessions, patients are seen on an
individual basis for an initial interview, diet consultation, and
instruction if necessary on insulin administration and self blood
glucose monitoring.
The department is affiliated with a diabetes supply center, Ross
Park Pharmacy, which is located on our East Campus. We bill all
major insurance companies for supplies.
The program is reimbursed by most major insurance companies including
Medicare/Medicaid. You must have a physician referral to enter this
program.
